Forms for Students



Students may apply for Academic Transcript/Testimonial/Course Outline from the University by the following methods:

Method 1: Application fee to be paid by Visa/MasterCard
Points to note:
1. An academic transcript is a complete and official record, semester-by-semester, of all the courses taken and grades received during your study at the University.
2. If you require your transcript to be evaluated by World Education Services (WES). Please choose "e-Copy Version" of Academic Transcript and read the Important Note 6 in the application form.
3. For "e-copy version of Transcript", applicants should apply for a "Student Copy" if the academic transcript is solely for personal use/retention. "Official Copy" is to be sent directly by the Registry's e-mail to the institutions or prospective employers.
4. A testimonial is a letter of certification of your study at the University excluding course assessment results (such as Proof of Student/Graudation Status, Proof of payment of tuition Fee, Proof of award of Scholarship.)
5. If you are requested for "Proof of Medium of Instruction", please apply for a "Testimonial" and select "Other" under "Type(s) of Testimonial that you requested". For other item(s) that you list under "Other" of the application for testimonial, please confirm with Registry Staff in advance if it is possible to be issued for you.

  • After submission of online application form, our Registry Staff will contact you via email within 2 working days to confirm whether your application is ready to be processed.

    Method 2: Application fee to be paid by a Cross-cheque
    Please download and fill in the application form in respective PDF version and submit the application form together with a cross-cheque by following the “Note” in the application form to Hong Kong Shue Yan University.

    Method 3: Application fee to be paid by Cash/Octopus
    Please download and fill in the application form in respective PDF version and submit the application form together with the application fees in cash/Octopus at Information Counter, G/F, Academic Building, Hong Kong Shue Yan University.

    A student who has completed at least one semester and who has earned a satisfactory assessment result for the semester may, in the case of serious illness with written attestation from a registered medical doctor, or due to any other exigent matter, apply to the Senior Registrar/Registrar for suspension of study IN PERSON for up to one academic year during designated office hours. Please contact your Departmental Administrative staff via email before submitting the application.

    If a student wishes to withdraw from his/her programme of study, he/she is required to submit the "Application for Withdrawal of Study" IN PERSON and complete the clearance procedures during designated office hours. Please contact your Departmental Administrative staff via email before submitting the application.

    The application for transfer of undergraduate programme is open from late June to mid-July each year. Students who are interested in transferring to another programme should submit the application form (Login required, please use your University login account) and supporting documents to your departmental administrative staff at Registry IN PERSON by July 15 before the commencement of next academic year.

    An application fee of HK$200 will be charged.

    Students who have lost their student card should report loss to the Registry and apply for a replacement of student card by submitting the application form(Login required, please use your University login account) to Information Counter (G/F, Academic Building) IN PERSON.

    An application fee of HK$100 will be charged.

    The normal processing time for an application for replacement of student card is 5 working days. The new student card should be collected immediately and would be discarded after 3 months (from date of collection). A handling fee is required when resubmission of application.


    Students who have any changes in their personal particulars should submit the form of "Change of Student Personal Details"(Login required, please use your University login account) IN PERSON to the relevant Registry staff at RHB 305 (Faculty of Commerce / Faculty of Social Sciences) or RHB 306 (Faculty of Arts / Postgraduate Programmes) during office hours.
